Conflict of Interest Form 2 Process - 2002 - 2003
The following notice has been sent to Deans and Faculty:
Every September 1, the Conflict of Interest Disclosure form associated with sponsored project applications (Form 2) is updated in accordance with instructions from the New York State Commission on Ethics.

This form must be filed by each investigator (as defined below) with each application for sponsored project support, whether to an external sponsor or to an internal support program.
The campus policy (at http://naples.cc.sunysb.edu/Admin/policy.nsf/pages/p209) defines "investigator" as follows:
The principal Investigator, co-principal Investigators and all other person(s) who are responsible for the design, conduct, or reporting of University programs, projects, activities or services described in an application or prospective application made through the University at Stony Brook for external and selected internal support, or in an award made to the University without application. For the purposes of this policy, "Investigator" shall include the Investigator's spouse and all dependent children.
The forms are to be reviewed and endorsed by the relevant Dean or Deans and forwarded to the Office of Research Compliance (Z = 3368). The Deans endorsement of the 4 page form (SUSB 299) accompanying an application indicates that all relevant forms have been filed, reviewed and that there are no unresolved conflicts.
Investigators are required to file updated forms if the circumstances of a prior form change.
